IAC Safety Director, Steve Johnson, has published the Transport Canada reports in reference to July's Pitts S-1S accident at Rocky Mountain House. He manages the IAC site: www.allairplanesbreak.com

His documents are fairly large, I've converted them to PDF and attached them here.

Two tragic items were contributing factors.

1. The backing out of the down elevator stop bolt.
2. Apparent lack of crossover spin training.
